Please Refer to the SNEYFC link below to find the current 2010 medical release form that ALL TRAVEL PLAYERS need as part of registration.  Once on the site, on the left hand side of the screen click on handouts, then 2010 medical release form.
 
 
The season is in full swing Monday through Friday at Guile field.  Mandatory weigh-ins are fast approaching coming August 28th.

     We will NEED to see an original birth certificate to verify your child's age.   A parental/guardian consent form must also be acknowledged on the on line registration.  

     A Southern New England Youth Football and Cheerleading (SNEYFC) medical consent form is also REQUIRED to be completed by your family doctor and then signed by the team head coach.
